PMVI Inspection Reports for Buses & Accessible Vehicles
Under the new PMVI regulation, technicians must fill out an inspection report in conjunction with the annual or semi-annual safety sticker or SSC. This is a new requirement that was not part of the previous regulation. These reports (which are a 2-part NCR snap-set) are available individually or in packages of 50.

- Single forms = $1.00 each
- 50 forms = $39.00
- 100 forms = $45.00
- 250 forms = $85.00
- 500 forms = $135.00
For stations that want to custom imprint their company name, address and, station number on each form, the prices are as follows:
- 50 forms = $54.00
- 100 forms = $60.00
- 250 forms = $103.25
- 500 forms = $159.00
- 1000 forms = 244.00
